Position Summary

We are seeking an experienced and dedicated ACT Clinical Manager to lead an Assertive Community Treatment (ACT) team. ACT is an evidence-based service model designed to provide comprehensive, person-centered mental health care to individuals with severe and persistent mental illnesses.

Under the supervision of the Clinical Director, the ACT Clinical Manager will oversee the daily operations of the ACT team, providing clinical guidance, managing intake and admissions, ensuring census stability, and leading a multidisciplinary team. This role requires strong leadership, problem-solving skills, and a commitment to delivering high-quality care while supporting both clients and staff.

Key Responsibilities

Direct and oversee the day-to-day operations of the ACT team, including scheduling and ensuring 24/7 service coverage.

Lead daily team meetings to evaluate client status, coordinate treatment activities, and address immediate client needs.

Establish and maintain partnerships with law enforcement, human services agencies, landlords, employers, and other community resources.

Provide ACT interventions and direct client support at least 50% of the time, working alongside team members to help clients reach their goals.

Assign new clients to team members, review intake information, and ensure the development of Individual Service Plans (ISPs) that align with client needs.

Ensure team documentation is completed accurately and in compliance with ACT model standards and regulatory requirements.

Serve as the Active Qualified Practitioner responsible for approving progress notes, Individual Recovery Plans, and Individual Specific Service Plans.

Provide on-call support for crisis situations and ensure team coverage during both scheduled and unscheduled hours.

Conduct quality assurance audits, review clinical records, and ensure effective service delivery and discharge planning.

Train team members on entitlement programs, housing support, Medicaid enrollment, and appeals for denied benefits (SSI, SSDI, VA).

Oversee clinical services for consumers in supported housing, ensuring service provision aligns with client needs.

Manage financial agreements for housing support and ensure timely rent payments.

Implement staff training and professional development programs to enhance team effectiveness.

Review and approve client documentation within the Electronic Health Record (EHR) system.

Supervise and support staff through coaching, mentorship, and performance management.

Qualifications & Experience

Education: Master's degree in a related field is required.

Licensure: Must be a licensed Mental Health Clinician in the District of Columbia (LICSW or LCPC).

Experience:

3-6 years of experience working with individuals with chronic medical conditions, serious mental health disorders, and/or substance use disorders.

2-4 years of experience in case management, crisis intervention, medication management, therapy, or rehabilitation support for adults.

Minimum of 2 years in a supervisory role (4 years preferred).

Technical Skills: Proficiency in Microsoft Outlook and other standard software applications.

Work Authorization: Must be legally authorized to work in the U.S.
